CICA ( Criminal Injuries Compensation Authority

I made a DSAR request to the CICA ( Criminal Injuries Compensation Authority ) towards the END of 2020 . Because I was told a claim I submitted in 2020 was “rejected due to duplicate” . Witch I dispute because I never made a claim before , I received information with a forged signature . And another application witch was raised at my ex girlfriends address . You can see from the attached witch is a police document I signed when I was a victim of a crime , I suggest this document is witch GMP relay for my permission for them to send the information over the the CICA . You can see I clearly DID NOT consent yes or no to the question. Witch asks “ do I give greater Manchester police permission to send information over to the CICA about the incident? Witch clearly this document proves I did not consent . This old claim witch my ex girlfriend submitted is preventing me getting compensation for injuries I substained through beings.l victim of a crime witch I believe is my right .
